What does an EHR cost? Prices of electronic health record systems for your practice

What does an electronic health record (EHR, in Dutch EPD) cost for your healthcare practice? Prices per practitioner, one-off costs and worked examples for 5 and 50 employees.

An EHR for your healthcare practice usually costs an amount per practitioner per month, plus one-off costs for activation and moving records. We list the price lists of three EHR suppliers and work out a small and a large practice.

What does an EHR cost, in short?

An electronic health record (EHR, in Dutch EPD) for a healthcare practice, such as a physiotherapy, mental health or GP practice, usually costs an amount per practitioner per month. On 1 October 2026 we went through the price lists of three EHR suppliers from our comparison that publish their prices. There an EHR costs €36 to €78,60 per practitioner per month, or with one supplier €29 per practice plus 0,7% of billed revenue.

For a practice with four practitioners, these three come to roughly €260 to €305 per month. One-off costs for activation, set-up and moving records come on top, from €0 to just over €700. Three suppliers is a small sample: most EHR suppliers work with a quote or a price per professional group. All amounts exclude VAT.

We only include prices that the supplier publishes itself. This is about patient record software in healthcare, not about environmental product declarations for building products, which are also abbreviated EPD in Dutch.

James Software gives starting prices that differ per professional group. For the other packages in our comparison, such as Incura and Epos, ask the supplier for the price.

What the price of an EHR depends on

  • Number of practitioners and their hours: most EHR systems charge per practitioner. James Software distinguishes by hours per week, so a part-timer costs less.
  • Professional group: a mental health practice has different records, billing rules and questionnaires from a physiotherapy practice. In mental healthcare Intramed charges €78,60 instead of €66,89 per care provider.
  • Revenue: with ZorgAdmin the price grows with billed revenue. That suits a small or new practice, a large practice less so.
  • Support users: a receptionist or practice manager counts with some suppliers. At James Software another user costs from €40 per month.
  • Extras: text message reminders, AI note-taking and integrations with an appointment service often cost extra. James Software, for example, charges €0,10 per text message.

Costs you easily forget

  • Moving records: switching from another EHR costs work and money. Ask what is moved: only client details, or also notes, measurements and claims.
  • Access after cancelling: medical records have to be kept for a long time. Ask what it costs to still view old records after you cancel.
  • Integrations: an integration with Vecozo, a calendar system, GP information systems or your accounting can cost extra per month.
  • Training: set-up and training are not always included in the price. Custom training costs €95 per hour at James Software.

Worked example: a practice with 5 and with 50 employees

We work with two fictional practices at one location. The small practice has four practitioners who work more than 24 hours a week, one practice assistant and €400.000 in billed revenue a year. The large practice has forty practitioners, ten support staff and €4 million in billed revenue. We take the prices for an existing practice from the table and calculate outside mental healthcare.

In this example the small practice pays roughly €3.150 to €3.650 a year, the large practice roughly €28.350 to €36.500. For Intramed we have not counted the practice assistants, because the price list charges per care provider; ask whether they count. For a large practice it pays to ask for a quote, because the price lists show starting prices.

In the earlier example from our comparison of EHR software, with two practitioners and €100.000 in revenue, ZorgAdmin at around €87 per month is clearly cheaper than Intramed at around €155. The higher the revenue per practitioner, the smaller that difference.

Price on request? How to compare quotes

Ask every EHR supplier for a quote for the same practice, with these items:

  • the number of practitioners per professional group and their hours per week;
  • whether support staff and interns count;
  • the one-off costs for activation, set-up, training and moving records;
  • which integrations are included, such as claims, calendar and text messages;
  • the cost of keeping access to records after cancelling;
  • the contract term, the notice period and the annual price indexation.
