You create a form at forms.google.com. For businesses, Google Forms is part of Google Workspace. That costs € 8,10 (Starter), € 16,20 (Standard) or € 25,30 (Plus) per user per month; with an annual contract that is € 6,80, € 13,60 and € 21,10. New customers get a temporary discount. You can try Google Workspace free for 14 days.
